Note that starting January 12, 2009 all Visa Waiver Program (VWP) travelers are<br \/>\nnow required to obtain a travel authorization via the <a href=\"https:\/\/esta.cbp.dhs.gov\/esta\/\">Electronic System for Travel<br \/>\nAuthorization<\/a> (ESTA) prior to traveling to the U.S. under the VWP.<br \/>\nPlease call your local U.S. consulate or embassy and inquire what the<br \/>\nrules are pertaining to your country.<\/li>\n<\/ul>\n<h3>Important notes for non-U.S. residents receiving travel support:<\/h3>\n<ul>\n<li>If you are already in the U.S. on a visa (e.g. F-1, J-1, or H-1B), please talk to the international office at your home institution and inquire about the rules pertaining to<br \/>\nreimbursement.<\/li>\n<li>All international visitors who will be receiving travel support are required to complete Vanderbilt&#8217;s online <b><a href=\"http:\/\/www.vanderbilt.edu\/gss\/enews\/questionnaire.php\">Business Visitor Questionnaire<\/a><\/b> (BVQ) prior to arriving on campus. This helps us determine whether Vanderbilt can avoid withholding tax from your reimbursement. Completing it before you arrive speeds up your<br \/>\nreimbursement.Please provide these answers to the following questions<br \/>\non the BVQ:<\/p>\n<ul>\n<li><b>Purpose of visit to Vanderbilt<\/b>: Use &#8220;Other&#8221; and enter<br \/>\n&#8220;Cumberland Conference&#8221;<\/li>\n<li><b>Vanderbilt Department you will visit<\/b>: Mathematics<\/li>\n<li><b>Department Contact Name<\/b>: KT Griffis<\/li>\n<li><b>Department Contact Email Address<\/b>:<br \/>\nKatherine.e.griffis@vanderbilt.edu<\/li>\n<li><b>Checkboxes<\/b>: Travel Costs Reimbursement<\/li>\n<\/ul>\n<\/li>\n<li>Please see the <a href=\"http:\/\/www.vanderbilt.edu\/gss\/immigration\/VisitorTaxCompliance.php\">tax information for business visitors<\/a> (including the BVQ).<\/li>\n<\/ul>\n<h3>Reimbursement procedures:<\/h3>\n<p>Please note the following.<\/p>\n<ul>\n<li>We will need original and detailed receipts for all expenses.<br \/>\nReceipts must indicate method of payment (including last four digits of<br \/>\ncard number for credit\/debit card payments).<\/li>\n<li>Reimbursements will be sent (by mail or direct deposit) after the<br \/>\nconference is over. Due to the University requirements for processing<br \/>\nthese, it is not possible to provide any reimbursement checks during the<br \/>\nconference.<\/li>\n<li>Vanderbilt University may be required to withhold U.S. income tax from your reimbursement.<\/li>\n<li>If you are a non-U.S. resident receiving travel support please see<br \/>\nthe instructions above. In particular, please fill out the online <b><a href=\"http:\/\/www.vanderbilt.edu\/gss\/enews\/questionnaire.php\">Business<br \/>\nVisitor Questionnaire<\/a><\/b> (BVQ) before arriving.<\/li>\n<\/ul>\n<p>To receive your reimbursement there are some forms you will have to<br \/>\nfill in. These will be available at the registration desk during the<br \/>\nconference, but you may wish to print them and fill them out in advance<br \/>\nto save time.<\/p>\n<ul>\n<li>Everyone receiving support must complete the <a href=\"https:\/\/cdn.vanderbilt.edu\/t2-my\/my-prd\/wp-content\/uploads\/sites\/2392\/2017\/05\/CumberlandConferenceReimbursementForm.pdf\">Cumberland Conference Reimbursement Form<\/a>.<\/li>\n<li>Everyone must fill in a tax form. U.S. citizens and permanent residents (green card holders) should fill in a <a href=\"https:\/\/cdn.vanderbilt.edu\/t2-my\/my-prd\/wp-content\/uploads\/sites\/2392\/2017\/05\/W-9.pdf\">W-9<\/a> form. Everyone else should fill in a <a href=\"https:\/\/cdn.vanderbilt.edu\/t2-my\/my-prd\/wp-content\/uploads\/sites\/2392\/2017\/05\/W-8BEN.pdf\">W-8BEN<\/a> form.<\/li>\n<li>Your payment can be direct-deposited to your bank account. If you<br \/>\nwant your payment deposited to a U.S. bank account, please fill in the<br \/>\n<a href=\"https:\/\/cdn.vanderbilt.edu\/t2-my\/my-prd\/wp-content\/uploads\/sites\/2392\/2017\/05\/ACHForm-Supplier.pdf\">Electronic Payment Information<\/a> form. If you want your payment wired to a bank outside the U.S., please fill in the <a href=\"https:\/\/cdn.vanderbilt.edu\/t2-my\/my-prd\/wp-content\/uploads\/sites\/2392\/2017\/05\/InternationalWireInformationForm.pdf\">International Wire Information<\/a> form.
