Minimal Transplant Survival Benefit for Hepatocellular Carcinoma: Is it Real or an Overestimation of Waitlist Life Expectancy?

We read with interest the article by Berry and Ioannou1 examining the net transplant survival benefit in patients undergoing liver transplantation (LT) for hepatocellular carcinoma (HCC) when compared to those without HCC based on analysis of the UNOS database. The authors defined transplant survival benefit as the difference between post-transplant survival and waitlist life expectancy and found a minimal benefit of LT in HCC patients (0.12 years/patient), far less than that observed in non-HCC patients (1.47 years/patient).
