Question: A 48-year-old woman presents with recurrent ascites and progressive weakness. She was discharged a little over a week ago and at that time multiple diagnostic studies, including a large volume paracentesis (serum ascites–albumin gradient [SAAG], 1.3), were done without a diagnosis to explain her ascites. She noted an increase in her abdominal size 4 months before her last admission. Her past medical history is notable for a recent pericardial effusion with window and pseudotomor cerebri with papilledema.
