Crohn’s and a Cough: Connecting the Dots

Question: A 22-year-old man with Crohn’s ileocolitis was referred for chronic cough and recurrent left lower lobe (LLL) pneumonia. He was diagnosed with Crohn’s ileocolitis 2 years ago, and briefly treated with oral mesalazine before being switched to azathioprine. He improved clinically, but approximately 1.5 years ago began experiencing a productive cough characterized by thick green sputum. He denied fevers, chills, and night sweats. He is a life-time nonsmoker and denied recent travel or sick contacts.
