Striking Finding on Colonoscopy: Is This a Watermelon Colon?

Question: A 50-year-old woman was referred for screening colonoscopy during small bowel transplant evaluation. One year ago, she developed acute mesenteric ischemia from superior mesenteric artery thrombosis. She required resection of a major portion of her small bowel, with terminal ileum kept in continuation with colon. There was no connection between the duodenum and the terminal ileum. Her colon was nonfunctional for 1 year and she was maintained on total parenteral nutrition. Routine laboratory studies were normal.
