Low FODMAP Diet for IBS

Staudacher et al1 have done important work on extending information on FODMAP restriction in subjects with diarrhea-predominant irritable bowel syndrome. There are 3 issues that require some additional attention. First, the investigators used as their primary endpoint “adequate symptom control,” in comparing the low FODMAP diet with the control (“placebo”) diet, and statistical significance was not met between the 2 diets. However, many of the secondary endpoints seemed to be highly significant (eg, bloating, flatulence, fecal urgency, and stool consistency), which is an important observation, and not surprising in diarrhea-predominant irritable bowel, if a low FODMAP diet is useful.
