Monitoring the Evolution of Portal Hypertension After Sustained Virologic Response

We read the article by Lens et al1 with great interest and would like to acknowledge the enormous collaborative effort underlying this important multicenter, prospective study coordinated by Drs Forns and García-Pagán. The authors investigated the impact of sustained virologic response (SVR) on interferon-free regimens on portal pressure. These regimens are highly effective and well-tolerated, even in patients who have already developed clinically significant portal hypertension (CSPH; hepatic venous pressure gradient [HVPG] ≥ 10 mm Hg),2 and thus are at considerable risk for (further) hepatic decompensation.
