P022 INVESTIGATING IBD TRANSITION READINESS AND FAMILY DYNAMICS: THE ADOLESCENT AND PRIMARY CAREGIVER PERSPECTIVE

Inflammatory Bowel Disease (IBD) is a chronic, immune-mediated disease that can lead to significant burden for both patients and families, with rates of childhood IBD rapidly increasing. Similar to other childhood chronic diseases, the transition between pediatric and adult care services can be challenging for patients and families and has been associated with poorer health outcomes immediately following transfer. This aspect of care is still a relatively new integration into IBD care and there are limited evidence-based approaches to appropriately support patients and their families during this transition.
