An unusual cause of chronic nausea and abdominal pain

Question: A 65-year-old woman presented for evaluation of postprandial nausea and abdominal pain of 5 years’ duration. The nausea was not associated with vomiting and was controlled with an antiemetic. The abdominal pain was predominantly located in the right upper quadrant; she had undergone a cholecystectomy decades before the onset of these symptoms. Owing to intractable pain, she had been started on daily opioid analgesics. She had a smoking history of >50 pack-years, and did not consume alcohol heavily.
