A Rare Cause of Positive Fecal Occult Blood Test

Question: A 50-year-old man positive for human immunodeficiency virus infection was admitted to the emergency department with a history of lethargy, dyspepsia, abdominal pain, and bloatedness, and occasional diarrhea for few months’ duration. There was a significant weight loss of 10 kg over 3 months duration. He denied any history of anorexia, vomiting, blood-stained stools, fever, chills, or flushing. No history of travel to South America or Africa was reported. On clinical examination, a few patchy white plaques were noted in the oropharyngeal mucosa.
