A Rare Cause of Hepatitis: “Think Outta the Box!”

Question: A 35-year-old black man with no past medical history presented to the emergency room with complaints of fever, abdominal pain, malaise, and vomiting, for a week. Physical examination was significant for a temperature of 103.2°F, pulse of 102 bpm, scleral icterus, and right upper quadrant tenderness to palpation. Laboratory work revealed aspartate aminotransferase of 531 U/L, alanine aminotransferase of 422 U/L, total bilirubin of 5.7 mg/dL, alkaline phosphatase of 262 U/L, platelets of 97,000/mm3, and international normalized ratio of 1.2.
