We thank the editors for giving us an opportunity to comment on Drs Kalman and Goldberg’s excellent summary of our paper on the use of obeticholic acid (OCA) in an experimental model of cirrhosis. Their commentary raises several issues regarding the relevance of the inflamed gut as a target for the management of portal hypertension and related complications in cirrhosis.
