Unusual Cause of an Umbilical Nodule

Question: A 48-year-old man who received a diagnosis of chronic hepatitis B 20 years ago but was not regularly followed up. He recently received a diagnosis of hepatocellular carcinoma (HCC). The initial presentation was poor appetite and abdominal pain, which had persisted for 3 months. The initial stage was T3N0M0 and the serum α-fetoprotein was 1760 ng/mL. The patient did not pursue follow-up and refused treatment. A progressive umbilical mass was noted 5 months later, and an umbilical mass with bleeding was observed (Figure A).
