Lack of Survival Benefit following Liver Transplantation With MELD Exception Points for Hepatocellular Carcinoma: Beyond the Unblinding of Lady Justice

The ever-increasing success of liver transplantation as a life-saving procedure has led to the demand for donor organs far outstripping supply. Nearly 1 in 5 patients waiting for a liver transplant in the United States dies before an organ becomes available.1,2 Allocating donor organs in a manner of optimal equity and utility has been a persistent challenge. Although the risk of dying on the waitlist varies geographically, it is unacceptably high in all regions of the United States: 2931 waitlisted patients died or became too sick to transplant between June of 2013 and June of 2014.
