Inter-provider communication has an important role in the transition of patient care and patient safety. Several studies have demonstrated that communication breakdown plays a significant role in the root cause of sentinel and adverse events in health care. The complexity and volume of patients on the inpatient gastroenterology service require that sensitive and critical information be communicated during hand-offs. Our aims were to develop a computerized Epic hand-off tool for the inpatient gastroenterology service to improve communication of patient information, while also improving efficiency and provider satisfaction by 2 points on the Likert scale.
